Why an intake form?  

The intake form gives me information about you, your quilt and your preferences.  The intake form lets you tell me about your quilt top, backing, batting, preferred quilting style and gives me an idea as to when you need your quilt completed. The intake form automatically adds you to my calendar and it gives me information so that I can contact you about your quilt.  

 At Prairie Patchwork Co. (Kim Boisson), we specialize in quality longarm quilting services tailored to your needs. We ensure timely service and customer satisfaction with a 2 to 3-week turnaround time.  Please note:  we quilt your top as we receive it.
